Cover
On admission
it is assumed that residents will remain registered with
their own GP. If this is not possible or the resident wishes
to change their GP, staff will advise on the availability
of local GPs and will help to arrange registration. Arrangements
have been made for routine medical cover and specialized
Occupational and Physiotherapists will visit on request.
Visits by physiotherapists can be required by GPs. Domiciliary
visits are made by dentists, chiropodists and opticians.
Admission
For
admission to Betsy Clara a resident requires a diagnosis
of Dementia that has become severe enough to require Nursing
Care. Referrals are taken from Private, Care Management
or Mental Health Teams. Once a bed is available a pre admission
assessment is completed to ensure that the prospective resident's
needs can be met at Betsy Clara. As far as possible, relatives
and other health professionals are involved in this process.
From
the actual day of admission, a comprehensive assessment
is commenced to ensure that all personal, health and spiritual
needs are assessed to ensure that the care planning stage
can begin. Should the resident have a current GP from an
area that is not covered by the Nursing Home, another GP
will be requested to cover all medical needs in accordance
with the Primary Care Trust Guidelines.
Once
the plan of care is implemented, this will be an ongoing
assessment of care needs and will be evaluated to ensure
that the resident's needs are current and have not changed.
Relatives
are invited to discuss the care of residents whenever they
have a concern. These requests are directed to the Trained
Nurses as these Team members are responsible for overseeing
all care needs. They will liase with GPs in regard to medication
and health issues and can request input from other members
of the Multi-disciplinary team such as Physiotherapists,
Dieticians or Psychiatric nurses.
Relatives
are involved in Care Planning for their relative and this
is reviewed each year or sooner should care needs change.
